Job Description

Mindify Wellness and Care is seeking a highly skilled, compassionate, and patient-centered Clinical Social Worker to provide comprehensive clinical services to our diverse client population. This role is an integral part of our collaborative care team , supporting individuals in achieving their mental wellness goals through evidence-based interventions and holistic care.

Position Overview

This is a hybrid position requiring both remote and in-person work, with travel among our adult day care centers across Northern and Central New Jersey . The CSW will work with individuals, families, and groups to enhance social and emotional well-being.

Essential Duties and Responsibilities

• Provide individual, group, and family therapy to clients with a range of needs, including mental health concerns, substance use disorders, and family challenges
• Conduct comprehensive psychosocial assessments and develop individualized treatment plans
• Deliver evidence-based interventions to support client progress and wellness goals
• Coordinate care with multidisciplinary providers, including psychiatrists, PMHNPs, psychologists, and nurses
• Advocate for clients and their families to ensure access to appropriate services and resources
• Provide education, guidance, and emotional support to clients and families
• Maintain accurate and timely clinical documentation and reports
• Participate in team meetings and collaborative care planning
• Contribute to the development of innovative social work interventions and programming
• Engage in ongoing professional development and training opportunities

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited program
• Active New Jersey (NJ) licensure required (e.g., LSW or LCSW)
• Minimum of 2+ years of experience providing clinical social work services
• Bilingual required (Spanish and English)
• Strong clinical assessment and therapeutic skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively within a multidisciplinary team
• Demonstrated commitment to social justice, diversity, equity, and inclusion
